The role of EBV in post-transplant malignancies: a review.

چکیده
Post-transplant lymphoproliferative disorder (PTLD) is a rare but frequently fatal complication of iatrogenic immunosuppression. PTLD encompasses a spectrum of B cell lymphoproliferations ranging from reactive plasmacytic hyperplasia to monomorphic B cell lymphoma. The tumours are almost always associated with Epstein-Barr Virus (EBV), and similar lymphoproliferative disorders also occur in other congenital and acquired immunodeficiency states. These malignancies reflect an imbalance in the normal control of EBV infection, although the virus is also linked to a subset of Hodgkin’s disease and T cell lymphomas in apparently immunocompetent individuals. Other EBV associated tumours occurring in particular areas of the world include Burkitt’s lymphoma in equatorial Africa and nasopharyngeal carcinoma in South East Asia. This review discusses the biology of EBV infection in the normal and immunocompromised host and the risk factors and pathogenesis of EBV associated PTLD.

متن کاملDe-Novo Malignancies Post Heart Transplantation: A Review of the Literature on the Mechanisms, Types, and Causes of the Malignancies
Background: Heart transplantation is a gold standard treatment for patients with end-stage heart disease. With the use of potent immunosuppressive agents and effective post-transplant management of heart transplant recipients, the survival rate of the patients and graft survival have significantly improved. One of the identified complications of heart transplantation is the development of post-...
متن کاملAsymptomatic Epstein-Barr Virus Shedding in the Urine of Kidney Transplant Recipients: Case Reports and Review of the Literature
Epstein-Barr virus (EBV) is associated with a wide range of malignancies and complications like post-transplant lymphoproliferative disorder (PTLD). To suppress active EBV infection in transplant recipients, who are at a heightened risk of developing PTLD, EBV DNAemia monitoring has been recommended. Quantitative multiplex real time polymerase chain reaction (QMRTPCR) offered the advantage of d...
متن کاملRecent advances in the risk factors, diagnosis and management of Epstein-Barr virus post-transplant lymphoproliferative disease.
Fifty years after the first reports of Epstein-Barr virus (EBV)-associated endemic Burkitt's lymphoma, EBV has emerged as the third most prevalent oncogenic virus worldwide. EBV infection is associated with various malignancies including Hodgkin and non-Hodgkin lymphoma, NK/T-cell lymphoma and nasopharyngeal carcinoma. Despite the highly specific immunologic control in the immunocompetent host,...
متن کاملEpstein-Barr virus-induced T cell lymphoma in solid organ transplant recipients.
Epstein-Barr virus (EBV) infection in transplant recipients can lead to lymphomas termed posttransplantation lymphoproliferative disorders (PTLDs). Most PTLDs are malignancies of B lymphocytes and are linked to EBV infection, but the rare T lymphocyte PTLDs have been inconsistently linked to EBV infection. Although the B lymphocyte is the main host cell of EBV, it has been suggested that T lymp...
